Additional charges may apply -Due to an increasingly heavy paperwork burden, effective October 1st, 2010, a $15 charge will be applied for all medication pre-authorizations performed outside of a scheduled office visit.A $25 charge will be applied for all missed appointments and appointments cancelled without 24 hour prior notice. For your convenience -Our office offers weekend and evening sick appointments and evening Well Woman and Well Child appointments.Walk-in appointments are being offered as follows:Effective Monday, January 12th, 2009, our office will be offering walk-in appointments (No appointment necessary) for established patients only as follows:Monday - Friday, 8:30 AM - 11:30 AM & 1:30 PM - 4:00 PM for the following: Cold Symptoms; Sore Throat, Cough, Fever; Sinus Symptoms; Flu Symptoms; Nausea; Vomiting: Skin Rashes; Minor Injuries; Muscles/Joint Sprains; Minor Lacerations; New Onset Back/Neck Pain. Providers will not address ongoing medical conditions/issues during walk-in appointments. Patients will be examined by an available provider. We can not guarantee this will be your usual provider. |
