Emergencies: If you are experiencing a medical emergency, you should dial 911 or proceed directly to the nearest hospital emergency room. Your insurance does not require a pre-authorization for emergency care. If you have an HMO plan, you should contact our office as soon as possible after your emergency room visit. |

New Patient Registration Procedures: Please arrive to the office 15 minutes prior to appointment to fill out registration forms. Bring all insurance information. |

Specialty Care Referrals Policy: Please allow a minimum of 2 business days to process referrals. Updated information must be on file. Patient may pick up referrals during regular office hours. We do not fax referrals. |

Prescription Refill Policy: Please allow a minimum of 1 business day for refills of routine medications. Routine prescriptions will be called into the pharmacy Mon-Fri from 8-5. Written presciptions can be picked up at the office during regular office hours. We do not fax prescription numbers to 800 numbers. |
